Medical Reports

Drivers with Class A, B, C, D, E and F licences must file additional medical reports as part of renewing their commercial driver’s licence

 

  • Every five years if they are under 46 years old
  • Every three years if they are between 46 and 64 years old
  • Annually if they are 65 years old and older

 

These requirements are in keeping with the recommendations of the Canadian Council of Motor Transport Administrators, which are the basis of the U.S./Canadian Reciprocal Agreem ent for comm ercial drivers crossing the Canada/ U.S. border. The ministry will send drivers medical forms 90 days before the due date. This will give you enough time to schedule an appointment with your physician or nurse practitioner and return the completed form by the due date. The form will be mailed to the address showing on your driver’s licence. If you move, you must update your address.

Medical Reports

Commercial drivers (Classes A, B, C, D, E and F) are required to submit a medical report when they apply for a licence. Commercial drivers are required to meet higher licensing standards because they drive longer hours, often in poor weather conditions, and they are subject to greater physical demands due to driving large heavy vehicles.

At an affordable cost, our team of qualified and licensed physicians perform the medical assessments that the Ministry of Transportation requires drivers applying for a commercial license (A, B, C, D, E, and F) in addition to drivers applying for G and M licenses who do not meet the standards for peripheral vision to obtain. Depending on the age of an individual, you may be required to submit a medical assessment form as frequently as every year to once every five (5) years. It is the patient’s responsibility to present themselves to a medical office when their medical exam is due. At our clinic, we provide access to driver medical exam services EVERY DAY – 6 days/week – with extended hours.

To ensure your assessment is completed efficiently, please arrive with the following:

  • The Medical Report Form: This is typically mailed to you by the Ministry of Transportation 90 days prior to your due date.

  • Valid Identification: Your current Driver’s License.

  • Visual Aids: If you wear glasses or contact lenses for driving, you must bring them for the vision test.

  • Medical History: A list of your current medications and any relevant medical history.

Yes. While our primary focus is the standard Ontario MTO form (SR-LC-080 05-02), we can accommodate forms for out-of-province, limousine, and taxi licensing. Please note that these non-standard forms are subject to a premium rate distinct from our standard pricing.

No. Our clinic specializes in the Standard Medical Report Form. We do not complete complex specialist forms such as the Substance Abuse Assessment Form, Neurological Assessment Form, or the Cardiovascular Disease Assessment Form. These require specific specialists outside the scope of a standard driver’s medical.

While commercial drivers require regular testing, Class G and M drivers typically only require a medical exam if they do not meet the standard peripheral vision requirements. In these cases, the Ministry requires a medical assessment to ensure it is safe for the individual to operate a vehicle.

We understand that professional drivers have busy schedules. Our clinic operates 6 days a week (Monday through Saturday) with extended hours to accommodate you. While the physical clinic is closed on Sundays, our Call Center remains open to assist with booking and inquiries.

Yes. Our goal is to be efficient and save you time. The $50 documentation portion of your fee covers the immediate completion of your paperwork. Once the physician has performed the physical exam, urine test, vision, and hearing assessments, you will leave with your documentation ready to be submitted to the Ministry.
